Applications/Forms

On January 1, 2012, the IRB fully implemented Protis - Protocol Information System - for on line submission [and tracking] of applications. For an introduction to the system and user instructions click here: http://help.protis.usu.edu

Email submission of the application forms found on this (IRB) website are no longer accepted.  The forms are made available as a reference only.

Click here to determine what type of application you should should submit through Protis

The review process typically takes 4 to 6 weeks for the General Application and 2 to 3 weeks for the exempt application or oral history application.  The Course Research Application and the Request for Determination of Non Human Subject Research Application will take less time.

It is expected that all researchers (including student research assistants) complete the required CITI training prior to the submission of an IRB application click here for CITI training.  To check your current IRB certification click here.

It is also expected that once an approval is obtained researchers will implement the project immediately.  If there is a delay in research of one month or more, please notify the IRB asap.

NOTE:  If an informed consent or letter of information is a necessary component of your project, be advised that final IRB approval cannot be given and research cannot begin until you have uploaded a signed copy to your Protis application

IRB Application Forms (which are no longer accepted through email but are posted here as a reference only)

Before submitting a General Application (through Protis), we suggest that you examine the Reviewer’s Checklist to see what the IRB will be looking for in the review of your application and to ensure that you’ve provided sufficient detail in your application
